Home Plan vs. Host Plan in a BlueCard Claim

BlueCard lets members of one Blue plan get care while traveling or living in another plan's service area. That introduces two plans into a single claim, with different roles.

Who Does What in a BlueCard Claim

Member eligibility

  • Home Plan: primary authority, maintains enrollment and ID data
  • Host Plan: none; retrieves eligibility data from the Home Plan

Benefit design

  • Home Plan: establishes coverage rules, limits, and exclusions
  • Host Plan: none; applies the Home Plan's benefit parameters

Provider network

  • Home Plan: none for local network administration
  • Host Plan: maintains the local provider network

Provider pricing

  • Home Plan: none; relies on the Host network's negotiated rates
  • Host Plan: applies local contracted rates and fee schedules

Provider corrected claim

  • Home Plan: receives corrected data through Host Plan routing
  • Host Plan: primary intake point for local provider corrected claims

Member benefit appeal

  • Home Plan: controlling administrative authority
  • Host Plan: generally not part of the member-initiated appeal workflow

Prior authorization

  • Home Plan: establishes medical-necessity criteria and issues determinations
  • Host Plan: may facilitate clinical data transfer from the local provider

Utilization review

  • Home Plan: reviews ongoing care against its own criteria
  • Host Plan: limited to coordinating local discharge planning

Network-gap request

  • Home Plan: approves in-network benefit levels for out-of-network care
  • Host Plan: evaluates local network adequacy to support the request

Provider peer-to-peer

  • Home Plan: conducts the clinical discussion with the treating provider
  • Host Plan: may coordinate only if the function is locally delegated

External review

  • Home Plan: bound by the applicable state regulator or federal IRO determination
  • Host Plan: generally not involved
