Cigna is a brand used across multiple operations, administrators, and delegated vendors. Before you use any deadline, form, or submission route, confirm which one actually handled your denial.
Why the Brand Name Isn't Enough
A Cigna logo on your insurance card doesn't establish the legal insurer, plan administrator, claims risk, utilization-management vendor, behavioral administrator, correct form, correct route, regulator, or external-review path. Those details come from the documents themselves.
What to Examine
Entity and Administrator Cards
General commercial medical coverage and administration. The exact entity and plan still matter.
May have state-specific grievance, appeal, and regulator rules, especially in California.
A legal entity associated with various commercial products. It doesn't by itself establish the full appeal procedure.
May administer behavioral-health member and provider processes separately from standard Cigna medical appeals.
May manage utilization review and provider post-decision options for selected services. The denial notice controls formal appeal routing.
May use separate Cigna routing. Don't use a core-Cigna destination without verifying the card and notice.
May indicate Cigna supplies network or administrative functions while another TPA or insurer handles other responsibilities.
State insurance rules may apply to the appeal and external-review process.
The employer or plan sponsor bears the claims risk. The Summary Plan Description and federal rules may be central.
Product Labels
Product names such as Open Access Plus, PPO, LocalPlus, HMO, EPO, SureFit, and Clearity describe a network or benefit design. They don't by themselves establish funding, entity, route, form, or state jurisdiction. Use the documents above, not the product name, to confirm those details.

California entity identification matters because the downstream regulator can differ. Cigna HealthCare of California, Inc. products generally point toward the Department of Managed Health Care, while Cigna Health and Life Insurance Company products generally point toward the California Department of Insurance. There is no single California route that applies to every Cigna product.
