Documents to Gather for a Humana Commercial Appeal

Representative and Minor-Member Documents

Appointment, PHI Disclosure, and Minor-Member Authority

Appointment as an appeal representative, permission to disclose PHI, and guardianship or custody authority may all be separate functions. For a verified Humana-controlled legacy case, the Appointment of Authorized Representative form is the supported instrument, and it requires a handwritten signature dated on or after the denial; electronic signatures are not accepted. For an ordinary minor claim, the child remains the member, and the parent or guardian signs the representative form indicating their relationship. Cases involving minor-consented care, reproductive care, behavioral health, substance-use treatment, contested custody, or unclear authority require legal or compliance review before records are disclosed or an appeal is transmitted.
