Deadline & Routing
This page explains how to find your answer, because no static answer applies to every Cigna commercial denial.
Direct Answer
Cigna's general commercial materials commonly refer to a 180-calendar-day appeal period, but that is not a universal rule for every plan, state, product, or appeal type. Use the deadline and starting event in the denial notice and controlling plan documents. There is also no single verified Cigna appeal fax or mailing address that safely applies to every commercial member appeal.
Where to Find the Deadline
Your first and most specific source for the deadline and starting event.
Often accompanies the denial and restates your filing window.
Especially important for a self-funded employer plan.
For a fully insured plan, this document carries the state-regulated benefit language.
For a post-service claim, the EOB often restates appeal rights.
An authenticated member account may display plan-specific deadline and routing information.
When documents conflict or are unclear, a phone confirmation can help, though it doesn't replace the written record.
What to Record
Date of the decision
Date you received it
The exact number of days
Whether those are calendar or business days
The event that starts the clock
The internal appeal stage
Whether the claim is pre-service, post-service, urgent, or concurrent
Any state-specific language
A separate external-review deadline, if one applies
Baseline vs. Exceptions
General Cigna materials commonly use 180 calendar days, and some plans may allow longer. California HMO materials use a materially different filing period, often referenced as one year from the denial notice. New York-regulated plans have specific published rules. Illinois external review is a separate stage with its own deadline. Self-funded plan documents may control instead of the general baseline. External-review deadlines are never the same as the internal appeal deadline. There is no public Cigna deadline calculator on this site because the applicable period depends on too many plan-specific variables.
How to Identify the Route
Whether you are the member or the provider
Whether the decision is medical or behavioral
Whether the card shows Cigna, GWH-Cigna, or "G"
Whether Shared Administration language appears
Whether EviCore was involved
Your state
Whether the plan is fully insured or self-funded
The appeal type
Which form applies, if any
Whether the route is fax, mail, portal, or another method
Whether representative paperwork is required
The Wrong-Fax Warning
A fax number printed on a Cigna privacy or representative form may be intended for the Central HIPAA Unit rather than the appeals department. The same number can also appear on California grievance and provider dispute materials for reasons unrelated to a standard commercial member appeal. This page does not display that number, and you shouldn't rely on it as a default appeal destination either. Use the route printed on your denial notice.
Commonly Confused With a Member Appeal
Proof of Transmission vs. Proof of Receipt
Recordkeeping
The complete packet
The appeal form or letter
Representative documents
The transmission record
A tracked-mail receipt
Portal confirmation, if applicable
Call notes
Later receipt confirmation
Any case or reference number issued by the plan
